If you are a healthcare professional interested in improving healthcare
transitions, then we have an opportunity for you!
Eric Coleman, MD, MPH, Professor of Medicine and Head of the Division of
Health Care Policy and Research at the University of Colorado at Denver, as well
as a nationally recognized care transitions expert, will present “Care
Transitions Workshop…It’s About the People.”
The workshop is hosted by Telligen
under contract with the Iowa
Department of Inspections & Appeals’ Health Facilities Division at the
Prairie Meadows Event Center in Altoona, Iowa on Thursday, May 31st,
2012. Registration and
continental breakfast will begin at 7:30 AM with the program to follow from 8:15
AM to 2:15 PM. Free lunch will also
be provided.
Session
topics include Challenging the Label “Non-compliant
Patient”; The Who, What,
When of Predicting Risk;
Medication Reconciliation with a Twist; and much more.
Unfortunately, no pharmacist CEU’s will be offered at this session.
