Starting July 1, 2012, Abbott Diabetes Care will be the single preferred manufacturer for diabetic blood glucose monitors and test strips for Iowa Medicaid.  These glucose monitors include:  FreeStyle Lite, FreeStyle Freedom Lite, Precision Xtra, and FreeStyle InsuLinx.  All other brands of blood glucose monitors and test strips will require prior authorization through the IME Medical Prior Authorization Unit at 888-424-2070 or 515-256-4624 locally in Des Moines.  To learn more, please read the informational letter provided by Iowa Medicaid.
