New Standardized Template of Medication Therapy Review for Medicare Part D

Starting January 1, 2013, all
MTM-eligible Medicare Part D patients who receive a medication review will get
three documents based on templates created by the CMS, including: cover letter,
medication action plan, along with a personal list of medications currently
being taken. This standardization was required by the CMS in order to increase
the quality of MTM services and ease the transition for patients who switch
Medicare Part D plans. All three documents can be given to the patient at the
appointment, but must be sent out within the CMS deadline of 14 days.
Standardization of
information provided by all MTM services will translate into more consistent
expectations for patients who receive these services. These expectations will
help define the role of pharmacists as medication experts, and may improve the
service’s public image in the future.
Pharmacists should expect to learn more about these various CMS
requirements from the MTM platforms (e.g.,
Mirixa, Outcomes, etc…), for which
current contracts exist to deliver appropriate care to Medicare Part D patients.
