Chronic Disease Management

Chronic Disease Management is an integrated care approach to managing illness, including screenings, check-ups, monitoring and coordinating treatment, and patient education. In turn, this helps improve quality of life as well as decrease costs for the patient.

IPA works closely with the Iowa Department of Public Health under the CDC 1815 program to provide new opportunities for pharmacies and pharmacists to enhance services to help patients with diabetes, hypertension and high cholesterol.


In November 2021, IPA hosted a webinar outlining diabetes care initiatives in the state, how to expand services in your practice setting, as well as funding opportunities through IPA and IDPH/CDC.
